Adding a second shower room is one of the few changes that genuinely alters how a house works day to day. It is also the job where the constraints are hardest to see from inside the room, because almost all of them are about where the waste can go and where the floor can be opened.

The soil connection comes first
A shower and basin can drain almost anywhere with a bit of ingenuity. A toilet cannot. It needs a connection to the soil stack with adequate fall over a sensible distance, and that single requirement decides most conversions before anything else is discussed.
Rooms that back onto an existing bathroom or sit directly above or below one are usually the easy candidates. Rooms on the far side of the house may still be possible, but the route becomes the main piece of work and needs to be established before any layout is drawn.

How small is too small
The honest test is not floor area but standing room. You need space to stand at the basin without your back against the shower, space to sit at the toilet without your knees touching anything, and a door that can open without hitting a fitting.
A narrow room can work extremely well if the fittings are chosen for it: a slimline basin, a wall-hung or short-projection toilet, and a shower enclosure with a sliding or bi-fold door rather than a hinged one. What does not work is a normal set of fittings squeezed into an abnormal space.

Ventilation and warmth in a room with no window
Many conversions produce an internal room with no window, which makes mechanical extraction the only route for the moist air. That fan needs a real duct to the outside, run as straight as possible, and a run-on so it keeps working after the room is vacated.
Warmth matters too. A small internal room heats quickly but also loses heat to unheated adjoining spaces. A towel radiator on the heating circuit, sized properly rather than chosen for looks, keeps the room from becoming the cold surface that everything else condenses onto.
- Duct the extractor to the outside, not into a loft or void.
- Use a run-on timer or humidity sensor rather than the light switch.
- Size the radiator for the room rather than picking the smallest that fits.
What you give up in the room next door
The space has to come from somewhere. Taking it from a bedroom can turn a double into something that no longer takes a double bed and two bedside tables, which affects both daily life and how the house is described when sold.
It is worth marking the new walls on the floor with tape and living with it for a couple of days before deciding. Most people either become certain very quickly or realise the room was better as it was, and both answers are cheap to reach at that stage.

Can a shower room go anywhere in the house?
Almost, but the practical answer is decided by the soil connection. Rooms near the existing stack are straightforward and rooms far from it need a route establishing before anything else is planned.
Is a shower room without a window acceptable?
It is common, and it depends entirely on the extraction being done properly. An internal room with a weak fan and a poor duct will show condensation problems quickly.
Does an extra shower room add value?
It changes how a house lives, which is the more reliable benefit. Whether it adds value depends on what it takes away, so the size of the room it comes out of matters as much as the new room itself.
